Our group home offers a consistent approach where the staff-to-client ratio allows more effective learning and truly individualized support. Days are structured but not clinical. Meals happen in a kitchen, laundry gets folded in a living room, and life happens the way it does in any other home.

Every client has an individualized, person-centered plan built around their goals — informed by the Support Intensity Scale (SIS) assessment. We focus on the skills that expand a person's independence: cooking, self-care, budgeting, transportation, community connection, and the everyday routines that make a house feel like a home.

Direct Support Professionals are trained, consistent, and present. Turnover matters, so we hire carefully and treat our team like the people they are.

What clients and families can expect

  • A person-centered plan that reflects the client's own words and goals.
  • Support with daily living skills — cooking, cleaning, self-care, medication management.
  • Community activities chosen by the client, not scheduled for them.
  • Regular communication with families and support coordinators.
  • Coordination with the CSB, primary care, and any behavioral or clinical supports.

What families and coordinators ask us most.

How do I get DD Waiver funding?

Start with your local Community Services Board (CSB). Your support coordinator can screen for waiver eligibility, add you to the wait list, and — once slots open — help match you with a licensed provider like Thorough Care. We're happy to walk families through this on a call.

What is the SIS assessment?

The Support Intensity Scale (SIS) is a standardized assessment that measures the type and intensity of support a person needs across daily life, health, safety, and community activities. We use SIS results to shape each client's individualized, person-centered plan.

How do I make a referral?

CSB support coordinators can refer directly by calling us or emailing info@thoroughcare.org. Families can also contact us — we'll coordinate with the CSB for you. Bring the client's ISP or SIS results if you have them; if not, we'll help gather what's needed.

Do you accept Medicaid?

Yes. Thorough Care is a licensed Medicaid DD Waiver provider in Virginia. Waiver services covered by Medicaid are billed directly — there's no out-of-pocket cost to the family for covered services.

Is there a waitlist?

It depends on the home and the level of support requested. Call us for current availability — we'll give you a straight answer and let you know timelines even if we're not the right fit today.
